Nursing Facility Transition

Independence is able to assist individuals with disabilities who reside in institutional settings to regain their independence within the community of their choice. We assist individuals with acquiring various levels of assistance based on individual need and individual wants.

Possible assistance includes:

• Assistance to develop a transition plan
• General assistance in finding affordable and accessible housing
• Peer support and advocacy
• Information and referral
• Independent living skills training and guidance.

What is considered an Institutional setting?

• Nursing Home
• Rehabilitation Facility
• Transitional or Supported Housing
• Prison

How long do I have to reside in the institution before I can get assistance?
Typically, Independence is able to assist people with the planning of the transition process. Contact Independence for more information.
